What are malar bags?
Malar bags is the term for bulging swellings on the cheekbone. These are fatty tissue and/or water accumulations that have slipped downwards due to weak muscles or a lack of skin elasticity. Visually, malar bags, also known as “cheek bags”, are similar to bags under the eyes, but they are higher and therefore closer to the eye.
What are the causes of malar bags?
Malar bags can have various causes: In addition to a genetic predisposition that comes to light as part of natural skin ageing, malar bags often occur when connective tissue is weak. Lifestyle also has an influence, as sun damage to the skin, an unhealthy diet, alcohol, nicotine, stress and little sleep can promote the development of malar bags. In some cases, they are a side effect of colds or allergies, very rarely a symptom of a more serious illness.
What is the difference between malar bags and tear bags?
At first glance, malar bags and lacrimal sacs look similar, which is why they are often confused by laypeople. However, the difference lies in the fact that lacrimal sacs are located directly under the eye, whereas malar bags form on the cheekbone, i.e. under the lacrimal sacs.
How does the treatment of malar bags work?
Injections of hyaluronic acid may be sufficient to treat slightly pronounced malar bags. With the help of targeted injections, the cheeks can be slightly plumped up to even out differences in volume. This makes the face look fresher and younger. Alternatively, there is the option of dissolving the fatty tissue with a “fat path injection”.
If the malar bag is very pronounced, these methods are usually not sufficient. In this case, surgery is required to remove the swelling. In this so-called extended lower eyelid lift, excess fatty tissue is removed through a fine incision at the level of the cheekbone and the skin on the cheeks is tightened. Sometimes an additional tightening of the muscles is useful, as this allows the entire cheek area to be lifted and rejuvenated. How long does the treatment of malar bags take?
The duration depends on the amount of treatment required and can vary from person to person. Experience shows that treatment in the form of an injection at the prevention center in Zurich takes around 20 minutes. An operation is more time-consuming, usually taking 60 minutes.
What do I need to consider after treating malar bags?
After the injection, you are immediately ready for social life again. Any slight redness or minor bruising at the injection sites can be easily concealed with a little make-up until they subside. You can support the healing process and reduce the risk of swelling by carefully cooling the affected area after the treatment.
After surgical removal of the malar bags, you should always carefully cool the affected area over the following days. Any wound pain that may occur can usually be treated with commercially available painkillers. After 5 days, you will have a follow-up appointment at the prevention center in Zurich, where the stitches will be removed. Allow yourself some rest until then. You should refrain from physical exertion such as sport for 3 weeks and from direct sunlight, solarium and sauna visits for at least 4 weeks.
When will I be socially acceptable again after the Malar Bags treatment?
This depends on the type of treatment. After an injection, you are immediately fit for social life again and can leave the prevention center immediately afterwards and resume your normal everyday activities. In the case of an operation, you should expect to be off work for 7-10 days.
What are the risks associated with the treatment?
The injection of hyaluronic acid or the fat transfer injection is associated with few risks. For example, slight redness, minor bruising or swelling may occur at the injection sites, but these will subside on their own within a short time. An occasional burning sensation at the injection sites is also only temporary.

However, as with all surgical procedures, especially when performed under general anesthesia, there are general risks such as post-operative bleeding, wound healing disorders and infections.
