Eyelid skin sagging mainly occurs in older people. However, it sometimes also occurs in relatively young people if there is stress, e.g. due to increased sun exposure or lack of sleep. It can affect both the upper and lower eyelid. On the upper eyelid, we speak of drooping eyelids, on the lower eyelid often of so-called bags under the eyes. However, the latter are caused by protruding orbital fat underneath the eyelid skin. Droopy eyelids do not always only cause aesthetic impairments, but can also create a disturbing sensation or cause a narrowing of the field of vision on the upper eyelid. These changes can be corrected by eyelid surgery. Lower eyelids can also be treated by laser in certain cases. A local anesthetic injection is usually administered during eyelid lift surgery.

Aim of the eyelid lift
The aim of an eyelid lift is often to improve aesthetics. After around three weeks, the consequences of the procedure are usually no longer particularly noticeable, but ultimately it is only possible to make a statement about the definitive success after several months. The result is not always as desired. It is therefore important to have a detailed discussion with the doctor beforehand. Further sagging at a later date cannot be prevented, meaning that another tightening procedure may have to be performed years or even decades later to restore the previous condition.
Before the eyelid lift
Every eyelid lift is preceded by a comprehensive consultation. During an individual consultation at the prevention-center Zurich, the goals of the procedure, the treatment process itself and the patient’s health history are discussed in detail.
Important: Smokers should refrain from smoking cigarettes and the like for several weeks before and after the operation, as otherwise wound healing will be impaired. Medication such as Aspirin® or Marcumar®, which inhibit blood clotting, may have to be discontinued in consultation with the medical team.
Treatment procedure
Upper eyelid
First, the incision line for removing the excess tissue is drawn on the eyelid skin. The incision is made in the eyelid crease or in the same direction and as much tissue as necessary is removed. In some cases, the eyelid muscles and the muscles that exert a downward pull on the eyebrows are also cut or severed. In order to maintain the position of the eyebrow, it can be fixed to the periosteum, a procedure known as “brow pexy”.

Upper eyelid lift
Lower eyelid from the outside
In the case of severely sagging tissue, the skin and eyelid muscle are cut horizontally. Excess fat and protruding skin are removed and the muscle is fixed to the bone envelope.
Lower eyelid from the inside
If so-called bags under the eyes are present, excess fatty tissue can be removed from the direction of the nose. However, this only works if the tissue is not too loose. The method is performed through incisions or with laser technology. The fatty tissue can also be pulled under the “dark circles” to make them disappear. Excess skin is removed, but the muscle is left intact, as otherwise a protruding eyelid edge with an overflowing eye can occur.

Lower eyelid lift
Additional tightening/fixation of the eyelid edge (canthopexy)
If the skin of the lower eyelid is severely sagging, it is sometimes necessary to tighten the edge of the eyelid or to fix the outer corner of the eyelid. The muscle is attached to the outer periosteum of the eye socket. After the respective procedure, the incision is sutured with thin thread material. In some cases, a temporary suture can also be placed between the upper and lower eyelid, possibly with a further traction thread upwards, to reinforce the effect. Pulling and fixing plasters are often applied.

After the eyelid lift
After the eyelid lift, the eyes are cooled to reduce swelling. Sometimes it is advantageous to apply a discreet adhesive strip. The client can see the doctor for stitches after about five days.
The customer should take it easy physically and avoid excessive head movements. Exposure to strong sunlight should be avoided at the beginning to prevent pigmentation disorders. Eyelid correction is often performed on an outpatient basis. The customer should be picked up after an outpatient procedure. It should be noted that for up to 24 hours it is not permitted to drive a car or operate machinery.
Risks & side effects
Eyelid lifting often results in bruising and swelling, but rarely pain. These symptoms almost always disappear within days to weeks. Sometimes bleeding and post-operative bleeding can also occur under the skin, so that further surgery may be necessary to stop or remove the blood. A feeling of tightness can sometimes remain. Injuries to the eye are very rare in eyelid corrections, but cannot be ruled out. Infections and inflammations, wound healing disorders, excessive scarring and nerve damage with numbness can also occur. Zug or nerve damage can sometimes cause the eyelid to tilt inwards or outwards or the eyelid to close insufficiently. Functional or cosmetic impairments cannot always be prevented. Allergic reactions of varying degrees of severity can also occur.

When is an eyelid lift an option?
An eyelid lift is an option for customers who are bothered by pronounced bags under the eyes or drooping eyelids. A distinction is made between two separate treatments: Bags under the eyes are corrected by a lower eyelid lift, drooping eyelids as part of an upper eyelid lift. If a client suffers from both conditions, these two procedures can be carried out simultaneously in one operation.
How long do the results of an eyelid lift last?
The results of eyelid surgery are usually permanent. Nevertheless, an unhealthy lifestyle (e.g. smoking, unhealthy diet) or the aging of the skin can lead to excess tissue in the eyes. If this is the case, the eyelid lift can be repeated without any problems.
What are the risks of an eyelid lift?
After the eyelid lift, the skin around the eyes may be red or swollen and there may be bruising. The result may be overcorrection, meaning that the eye can no longer be closed completely. However, this is tested by us during the operation and can therefore be avoided. The rarest risk of an eyelid lift is blindness due to post-operative bleeding. We prevent this by providing a professional blood supply during the operation and also as part of conscientious preparation and follow-up. The eyes may still be slightly sensitive to light after the operation. Women may also notice that their eyes still feel a little numb at the lash line when applying make-up. However, these symptoms are harmless and temporary.
Procedure for an eyelid lift
How exactly does an upper eyelid lift work?
When lifting the upper eyelid, it is important to know that we mainly work in the eyelid crease under the skin. Even if this area appears very thin, there is fat and muscle tissue in this region. After administering a painkiller, we mark on the eye what is to be removed later. We then induce local anesthesia and cut away the marked area. During the entire treatment, we ensure that the bleeding is properly stopped. A narrow strip of muscle and the excess fatty tissue are then removed, which is barely noticeable to the client. The scar is then placed precisely in the crease of the eyelid so that it will be difficult to see later. The natural shape of the eye is not altered, but the eye appears more alert and refreshed after the upper eyelid lift.
What happens during a lower eyelid lift?
With a lower eyelid lift, there is often more anxiety as the incision is made around 1 – 2 millimetres below the eyelashes. However, as the skin around the eyes is very thin, it heals fantastically, so you don’t have to worry about any noticeable scars later on. During the operation, excess tissue is removed so that the bags under the eyes disappear. The skin can also be lifted with a thread in the area of the outer corner of the eye and thus tightened. The lower eyelid lift takes about an hour and after 5 days the sutures are pulled in the prevention center.
Aftercare of an eyelid lift
What do I need to consider after an eyelid lift?
After an eyelid lift, it is particularly important that you give yourself and your body a break. You should take it easy physically for about a week, or 10 days in the case of a combined lift. In the first three days, it is advisable to sleep a little more and reduce the time spent in front of cell phone or computer screens.
When will I be socially acceptable after the eyelid lift?
After the eyelid lift, there may initially be minor swelling or bruising in the treatment area. These will subside on their own within a few days or after a week at the latest, so that the client is socially acceptable again.
When can I do sports again after the eyelid lift?
Following the eyelid lift, the patient should initially refrain from sport and take it easy physically. About two weeks later, light sporting activities can be resumed and slowly increased.
When can I apply make-up to my eyes again after an eyelid correction?
One week after the eyelid correction, the wound has healed and make-up can be carefully applied to the eye area again.
When can I return to work after the eyelid lift?
When you can return to work after an eyelid lift depends entirely on your profession, but you should allow at least one week for recovery. This also applies to people who work at home. Jobs where you have a lot of contact with other people can be resumed after about two weeks.
When can I fly again after an eyelid lift?
You can generally fly again the next day. However, you should only fly short distances, as longer flights are not conducive to wound healing and can lead to more severe swelling.
When are the stitches removed after an eyelid lift?
Normally, the stitches can be removed after 4 – 7 days. In our clinic, the stitches are usually removed 5 days after the procedure.
Financing
Does health insurance cover the costs of an eyelid lift?
Health insurance only covers the costs of medically indicated procedures, i.e. if there is a medical necessity for the procedure. As this is rarely the case with a lower eyelid lift, the costs are generally not covered. The situation is different in the case of an upper eyelid lift: drooping eyelids can be so pronounced that they significantly restrict the patient’s field of vision. If this can be professionally proven, the costs may be covered by health insurance.
